Remote Conditional Voter Registration Form

Use this form to notify our office that you wish to use a remote conditional voter registration, and complete a ballot remotely. If you have not already submitted an online voter registration, please do so at ocvote.gov/register . After you submit this form, our office will process your registration and notify you of the next steps required to complete the process.
